COVID-19 Information for Mosaic Patients
Mosaic Medical’s School-Based Health Centers are the result partnerships with the school district, county health department and Mosaic Medical. At each of our School-Based Health Centers (SBHC) we provide comprehensive, quality healthcare that is convenient and affordable. We do not turn away patients based on their ability to pay for medical services. We bring the doctor’s office right to the school, so students can be seen without missing school. Both students at the school and other children ages zero to 18 years are eligible for service, regardless of income, insurance or lack of insurance.
School-Based Health Services
Diagnosis and treatment of injuries and acute and chronic illness.
Primary pediatric care in the medical home model.
Routine physicals or well-child exams and preventative care.
Nutrition and weight education and management
Mental Health Care
Mental health screenings and assessments.
In some location we provide integrated behavioral health consultants who provide counseling and support from the primary medical home.
Co-located assessment and counseling through ongoing individual, family, and group therapy.
Referral when appropriate to other behavioral health services.
Oregon Health Plan (OHP) enrollment assistance
Referral coordination (when needed)
We offer same day appointments, late clinic hours and 24-hour on-call providers.
Payment – We Work with You
