Crook County School-based Health Center

Crook County School-based Health is a partnership among the Crook County Health Department, the Crook County School District, and Mosaic Medical. All Crook County public, private, and home-schooled students age 5-18 can receive their services at the school-based health center.

The health center offers physician and registered nurse care for well child checks (which include sports physicals and immunizations), acute (such as a cold) and chronic illness (such as diabetes and asthma) care, medication administration, health screening, as well as emotional health services.

The health center also offers Healthy Kids enrollment services on-site.

The school-based health center accepts private and Healthy Kids insurance and offers sliding-scale discount services to uninsured families.

The first Crook County School-based Health Center is conveniently located on the Crooked River Elementary School campus at 757 NE 1st Street in Prineville, Oregon.

Tuesday 8am-1pm (RN available), Wednesday 12-5pm (physician avaialble), Thursday 8am-1pm (physician available)
