What To Expect

At Mosaic Medical, we practice whole-patient care. Professional, compassionate care for you and your family is our bottom line. We strive to make your time in our clinic with our staff and providers as efficient, effective, and enjoyable as possible. We pride ourselves on providing access to all patients. We employ bilingual front desk staff at each of our clinics and offer on-site Spanish-language Medical Interpretive Services in addition to all other languages using a phone-based interpretive service.

The following describes what you can expect to experience when you visit one of our clinics as a new patient.

Check-In

  • When you arrive at your Mosaic Medical clinic, you will check-in with a receptionist at the front desk, letting them know you are ready for your appointment.
  • As a new patient, you will receive paperwork that must be completed prior to your visit.
  • You will have plenty of time to complete the paperwork while you're waiting to be called back.

Payment

Private/Commercially-Insured Patients. We can collect your co-payment by cash, Visa, Mastercard, or personal check at check-in. 

Medicare-Insured Patients. We can collect your 20% co-payment by cash, Visa, Mastercard, or personal check at check-in.

Medicaid-Insured Patients. We can collect any co-payments by cash, Visa, Mastercard, or personal check at check-in.

Underinsured or Uninsured Patients. After you complete a sliding scale application, we can collect your payment by cash, Visa, Mastercard, or personal check. All under and uninsured patients receive a 20% discount when paying their bill at the time of his or her visit. We ask that your payment be at least $20.00 toward your bill at the time of visit. When you come for your appointment, you will have the opportunity to meet with our outreach personnel to talk more about subsidized or free insurance options for you and your family. If your income qualifies you to apply for one or more of these programs, your Mosaic Medical Outreach worker can walk you step-by-step through this process.

At Mosaic Medical, access to quality health care services is at the heart of our organization’s mission. We believe that no person should forgo necessary medical care because of cost. If you are unable to pay your bill in full at the time of your visit, we will work with you to create a payment plan that takes your whole picture into account.

Your Visit

  • You will be called from the waiting room by a Registered Nurse or a Medical Assistant who works directly with your Mosaic Medical primary care provider.
  • Before seeing your physician, physician assistant, or nurse practitioner, your vital signs (often in addition to height and weight) will be taken and you'll share the reason for your visit.
  • You will meet with your primary care provider regarding the reason for your visit and your health history.
  • Your provider may refer you to an Outreach Coordinator or a Medication Assistance Coordinator. You may meet with them after your visit is finished or schedule another time to visit these offices.

Check-Out

  • If you are uninsured or for any reason were unable to make your co-payment at check-in, you should stop by the front desk before leaving the clinic.
  • Here, front desk personnel will determine the charges for your visit and help you make payment arrangements
  • At this time, you can also schedule any follow-up appointments advised by your primary care provider.
